normal align=left>三、投標人資格條件
normal align=left>所有投標人須滿足以下資格要求：
normal align=left>3.1在中華人民共和國境內合法注冊的，具有獨立企業法人資格的設備制造商或經銷商；
normal align=left>3.2具有合法經營資格的醫療器械經營許可證（在備經營范圍內）；
normal align=left>3.3投標人若為代理商時，其中：1標段的1.肺功能檢測儀、3.神經血管治療儀、4.糖尿病足篩查診斷箱；3標段的5.視頻腦電圖儀；4標段的1.周圍自主神經病變無創早期檢測系統、4.胰島素泵、5.氬氣高頻電刀、6.纖維支氣管鏡，若經銷商參加投標必須具有生產廠家針對該項目所出具的加蓋公章的授權文件原件及售后服務承諾函（不接受同一品牌同一型號的兩家及以上的授權文件）；
normal align=left>3.4投標單位近三年投標過程中無不良記錄及延期交貨情況；參加政府采購活動前三年內，在經營活動中沒有重大違法記錄；
normal align=left>3.5參與投標競爭的企業須開具投標單位所在地或項目所在地檢察機關出具的無行賄犯罪檔案查詢結果告知函（且顯示無行賄記錄開具時間為招標公告發布日之后）；
normal align=left>3.6 投標人近年度財務狀況報告（2015年度或2016年度財務審計報告，必須提供整本，財務審計報告須有會計師的簽字和蓋章，若投標人公司成立時間不足六個月，可不提供經審計的財務審計報告，但須有公司財務報表）及近半年內任意連續三個月依法繳納稅收和社會保障資金的相關證明材料；
normal align=left>3.7投標貨物必須符合國家標準、行業標準和專業標準等相關標準。并符合中華人民共和國國務院令第650號修訂后的《醫療器械監督管理條例》相關規定。
3.8根據《關于在政府采購活動中查詢及使用信用記錄有關問題的通知》(財庫[2016]125號)和豫財購【2016】15號的規定，對列入失信被執行人、重大稅收違法案件當事人名單、政府采購嚴重違法失信行為記錄名單的供應商，拒絕參與本項目采購活動；【查詢渠道：“信用中國”網站（www.creditchina.gov.cn）、中國政府采購網（www.ccgp.gov.cn）】
